ROADMAP AND TIMELINE for completion of  I-693 form: 

START!

1) Call us or message above to schedule an appointment .
2) Dr. Raus will review carefully your  records  and vaccines  during appointment and we will clearly  instruct you about  what vaccines or laboratory tests are needed to complete per current CDC/USCIS guidelines. 

EASY!

3) When lab results are completed you will be informed promptly via email/text or phone.
4) After completing the blood work and vaccinations , will  be able to complete the I-693 form. We have option for you to  fill out this form online. 

DONE!


5) Will meet you in the office for signature of the form.
6) You will receive sealed envelope with the original form signed.

A copy of the form will be emailed to you for your records.

7) you are sending the sealed envelope to your Lawyer or Immigration office per USCIS instructions. 

 FAQ

(1) Tuberculosis (TB) – For all applicants 2 years of age and older.
You will be screened with tuberculosis blood test- Quantiferon  TB gold.
For this test, a blood sample is required.
(2) Syphilis - for adult applicants 18-44 yo
This require a blood test.
(3) Gonorrhea urine test and Other communicable diseases will be assessed by civil surgeon - for adult applicants 18-24 yo 

We will ask you detailed questions about your previous and current medical conditions. Your medical records if available will be reviewed. You will be evaluated for major physical or mental disorders with associated harmful behavior, drug abuse or addiction, and other medical conditions. The civil surgeon will ask you questions , will take your vitals  and conduct a medical exam to ensure that applicants are not posing a threat to the public health of this country, to themselves or to others. 

 USCIS/CDC has specific criteria to determine which vaccines people applying for adjustment of status for US permanent residence (hereafter referred to as applicants) are required to show proof of having received.

* By USCIS Policy all laboratory tests should be ordered by authorized civil surgeon, NOT BY YOUR PRIMARY CARE PHYSICIAN 
You could use your insurance for the blood work ( verify with them the coverage and costs)  OR we could complete that part in our office through independent lab for $100/ set of labs.
